【應(ying)用(yong)】沙(sha)眼(yan)進(jin)行期(qi)、春(chun)季卡他性結(jie)(jie)膜(mo)炎(yan)、慢性濾泡性結(jie)(jie)膜(mo)炎(yan)等病,屬風熱相搏,血滯瘀結(jie)(jie)者(zhe);亦用(yong)于(yu)沙(sha)眼(yan)性角(jiao)膜(mo)炎(yan)。

犀角赤芍當歸尾,泄熱化(hua)瘀效相(xiang)當。

【附方】(1)止痛沒藥散(《醫宗(zong)金鑒》)沒藥(yao)60g,芒硝(xiao)45g,大(da)黃45g,血竭(jie)30g,搗(dao)篩(shai)為(wei)細末,食后(hou)熱(re)茶清調下(xia)3g。可(ke)改作湯劑(ji) :血竭(jie)2g(研末、藥(yao)液沖(chong)服(fu)),余(yu)藥(yao)各10g(芒硝(xiao)沖(chong)服(fu)),水煎服(fu)。功能(neng)瀉火,化瘀(yu)(yu),定痛。應用(yong)于(yu)眼外傷,屬(shu)血熱(re)瘀(yu)(yu)結,痛甚,便秘(mi)者尤宜。

【來源】民間方
【原料】人參20克 枸杞子350 克 熟地100 克 冰糖400 克 白酒10公斤
【制作】 將人參烘烤切片,枸杞除去雜質,用紗布袋裝上扎口備用。 冰糖放入鍋中,用適量水加熱溶化至沸,煉至色黃時,趁熱用紗布過濾去渣備用。白酒裝入酒壇內,將裝有人參、枸杞的布袋放入酒中,加蓋密封浸泡10-15天,每日攪拌1 次,泡至藥味盡淡,取出藥袋,用細布濾除沉淀物,加入冰糖攪勻,再靜置過濾,澄明即成。
【功效】強壯抗老,補陰血,烏須發,壯腰膝,強視力,活血通經。適用于病后體虛及貧血、營養不良、神經衰弱、糖尿病等患者飲用。無病常飲,亦有強身益壽之功。
【用法】飲用時,可根據自己的酒量,每次飲10-12克

人參補陽湯用于治傷寒余邪不散,上走空竅,其目隱澀赤脹,生翳羞明,頭痛骨痛。

【正文】
功用:清熱解毒,疏風散邪。
用法:水煎服。
禁忌:斟酌。
方解:本方主治大頭瘟(原書稱大頭天行),乃感受風熱疫毒之邪,壅于上焦,發于頭面所致。風熱疫毒上攻頭面,氣血壅滯,乃致頭面紅腫熱痛,甚則目不能開;溫毒壅滯咽喉,則咽喉紅腫而痛;里熱熾盛,津液被灼,則口渴;初起風熱時毒侵襲肌表,衛陽被郁,正邪相爭,故惡寒發熱;舌苔黃燥,脈數有力均為里熱熾盛之象。疫毒宜清解,風熱宜疏散,病位在上宜因勢利導。疏散上焦之風熱,清解上焦之疫毒,故法當解毒散邪兼施而以清熱解毒為主。方中重用酒連、酒芩清熱瀉火,祛上焦頭面熱毒為君。以牛蒡子、連翹、薄荷、僵蠶辛涼疏散頭面風熱為臣。玄參、馬勃、板藍根有加強清熱解毒之功;配甘草、桔梗以清利咽喉;陳皮理氣疏壅,以散邪熱郁結,共為佐藥。升麻、柴胡疏散風熱,并引諸藥上達頭面,且寓“火郁發之”之意,功兼佐使之用。諸藥配伍,共收清熱解毒,疏散風熱之功
清胃散

【主治】
胃火牙痛。牙痛牽引頭疼,面頰發熱,其齒喜冷惡熱,或牙宣出血,或牙齦紅腫潰爛,或唇舌腮頰腫痛,口氣熱臭,口干舌燥,舌紅苔黃,脈滑數。(本方常用于口腔炎、牙周炎、三叉神經痛等屬胃火上攻者。)
【正文】
功用:清胃涼血。
用法:上藥為細末,都作一服,水一盞半,煎至七分,去滓,放冷服之(現代用法:作湯劑,水煎服)。
禁忌:牙痛屬風寒及腎虛火炎者不宜。
方解:本方證是由胃有積熱,循經上攻所致。足陽明胃經循鼻入上齒,手陽明大腸經上項貫頰入下齒,胃中熱盛,循上沖則口氣熱臭;胃為多氣多血之腑,胃熱每致血分亦熱,血絡受傷,故牙宣出血,甚則牙齦潰爛;口干舌燥,舌紅苔黃,脈滑數俱為胃熱津傷之候。治宜清胃涼血。方用苦寒瀉火之黃連為君,直折胃腑之熱。臣以甘辛微寒之升麻,一取其清熱解毒,以治胃火牙痛;一取其輕清升散透發,可宣達郁遏之伏火,有“火郁發之”之意。黃連得升麻,降中寓升,則瀉火而無涼遏之弊;升麻得黃連,則散火而無升焰之虞。胃熱盛已侵及血分,進而耗傷陰血,故以生地涼血滋陰;丹皮涼血清熱,皆為臣藥。當歸養血活血,以助消腫止痛,為佐藥。升麻兼以引經為使。諸藥合用,共奏清胃涼血之效,以使上炎之火得降,血分之熱得除,于是循經外發諸癥,皆可因熱毒內徹而解。
瀉白散

本方主治肺有伏火郁熱之證。肺主氣,宜清肅下降,火熱郁結于肺,則氣逆不降而為喘咳;肺合皮毛,肺熱則外蒸于皮毛,故皮膚蒸熱;此熱不屬于外感,乃伏熱漸傷陰分所致,故熱以午后為甚,其特點是輕按覺熱、久按若無,與陽明之蒸蒸發熱、愈按愈盛者有別;舌紅苔黃,脈象細數是熱邪漸傷陰分之候。治宜清瀉肺中郁熱,平喘止咳。方中桑白皮甘寒性降,專入肺經,清瀉肺熱,平喘止咳,故以為君。地骨皮甘寒入肺,可助君藥清降肺中伏火,為臣藥。君臣相合,清瀉肺熱,以使金清氣肅。炙甘草、粳米養胃和中以扶肺氣,共為佐使。四藥合用,共奏瀉肺清熱,止咳平喘之功。
本方之特點是清中有潤、瀉中有補,既不是清透肺中實熱以治其標,也不是滋陰潤肺以治其本,而是清瀉肺中伏火以消郁熱,對小兒“稚陰”之體具有標本兼顧之功,與肺為嬌臟、不耐寒熱之生理特點亦甚吻合。

【正文】
功用:清瀉肝火,降逆止嘔。
用法:為末,水泛為丸,每服2-3克,溫開水送服。亦可作湯劑,用量參考原方比例酌定。
禁忌:斟酌。
方解:
本方證是由肝郁化火,橫逆犯胃,肝胃不和所致。肝之經脈布于脅肋,肝經自病則脅肋脹痛;犯胃則胃失和降,故嘈雜吞酸、嘔吐口苦;舌紅苔黃,脈象弦數乃肝經火郁之候。《素問?至真要大論》說:“諸逆沖上,皆屬于火”;“諸嘔吐酸,暴注下迫,皆屬于熱。”火熱當清,氣逆當降,故治宜清瀉肝火為主,兼以降逆止嘔。方中重用黃連為君,清瀉肝火,使肝火得清,自不橫逆犯胃;黃連亦善清瀉胃熱,胃火降則其氣自和,一藥而兩清肝胃,標本兼顧。然氣郁化火之證,純用大苦大寒既恐郁結不開,又慮折傷中陽,故又少佐辛熱之吳茱萸,一者疏肝解郁,以使肝氣條達,郁結得開;一者反佐以制黃連之寒,使瀉火而無涼遏之弊;一者取其下氣之用,以和胃降逆;一者可引領黃連入肝經。如此一味而功兼四用,以為佐使。二藥合用,共收清瀉肝火,降逆止嘔之效。
本方的配伍特點是辛開苦降,肝胃同治,瀉火而不至涼遏,降逆而不礙火郁,相反相成,使肝火得清,胃氣得降,則諸癥自愈
竹葉石膏湯

竹葉6克,石膏50克,半夏9克,麥門冬20克,人參6克,粳米10克,甘草6克。
【主治】
傷寒、溫病、暑病余熱未清,氣津兩傷證。身熱多汗,心胸煩悶,氣逆欲嘔,口干喜飲,或虛煩不寐,舌紅苔少,脈虛數。(本方常用于流腦后期、夏季熱、中暑等屬余熱未清,氣津兩傷者。糖尿病的干渴多飲屬胃熱陰傷者,亦可應用。)
【正文】
功用:清熱生津,益氣和胃。
用法:上七味,以水一斗,煮取六升,去滓,內粳米,煮米熟,湯成去米,溫服一升,日三服。
禁忌:本方清涼質潤,如內有痰濕,或陽虛發熱,均應忌用。
方解:
本方證乃熱病后期,余熱未清,氣津兩傷,胃氣不和所致。熱病后期,高熱雖除,但余熱留戀氣分,故見身熱有汗不解、脈數;余熱內擾,故心胸煩悶;口干,舌紅少苔是陰傷之兆;氣短神疲,脈虛是氣虛之征;胃失和降,乃致氣逆欲嘔。氣分余熱宜清,氣津兩傷宜補。治當清熱生津,益氣和胃。方中竹葉配石膏清透氣分余熱,除煩止渴為君。人參配麥冬補氣養陰生津為臣。半夏降道和胃以止嘔逆為佐。甘草、粳米和脾養胃以為使。全方清熱與益氣養陰并用,祛邪扶正兼顧,清而不寒,補而不滯,為本方的配伍特點。本方實為一首清補兩顧之劑,使熱清煩除、氣津得復,諸癥自愈,正如《醫宗金鑒》說:“以大寒之劑,易為清補之方。”
本方由白虎湯化裁而來。白虎湯證為熱盛而正不虛,本證為熱勢已衰,余熱未盡而氣津兩傷。熱既衰且胃氣不和,故去苦寒質潤的知母,加人參、麥冬益氣生津,竹葉除煩,半夏和胃。其中半夏雖溫,但配入清熱生津藥中,則溫燥之性去而降逆之用存,且有助于輸轉津液,使參、麥補而不滯,此善用半夏者也。
本(ben)方在(zai)《傷寒論》中(zhong)治“傷寒解(jie)后(hou),虛瀛(ying)少(shao)氣(qi),氣(qi)逆欲吐(tu)”證。在(zai)實際運用(yong)中(zhong),凡熱病過(guo)程中(zhong)見氣(qi)津已(yi)傷、身熱有汗(han)不退、胃失和降等均可使(shi)用(yong)。對于暑溫病發(fa)熱氣(qi)津已(yi)傷者,尤為(wei)適(shi)合
