柴胡疏肝散加減治療胃脘痛

胃(wei)(wei)(wei)脘痛屬(shu)于肝(gan)氣(qi)(qi)(qi)(qi)犯胃(wei)(wei)(wei)、胃(wei)(wei)(wei)失(shi)和(he)降之證者,治以疏(shu)肝(gan)和(he)胃(wei)(wei)(wei)為法。肝(gan)主疏(shu)泄,具有升發疏(shu)泄的作用(yong),肝(gan)氣(qi)(qi)(qi)(qi)條達能舒暢全身氣(qi)(qi)(qi)(qi)機(ji),如(ru)精神抑郁,悲哀惱怒,郁傷肝(gan)氣(qi)(qi)(qi)(qi),肝(gan)氣(qi)(qi)(qi)(qi)郁結,橫逆(ni)犯胃(wei)(wei)(wei),導(dao)致消化(hua)機(ji)能紊亂。癥見胸脅脹滿(man)、喜(xi)長嘆息、胃(wei)(wei)(wei)脘脹滿(man)疼痛、噯(ai)氣(qi)(qi)(qi)(qi)吞酸、嘈雜或嘔惡等。方(fang)用(yong)柴胡疏(shu)肝(gan)散加減,處方(fang):柴胡、白(bai)芍、枳實、川楝(lian)子(zi)、香附、陳皮、白(bai)術、甘(gan)草(cao)。方(fang)中(zhong)柴胡疏(shu)肝(gan)散結,枳實寬中(zhong)下(xia)氣(qi)(qi)(qi)(qi),枳實與柴胡同用(yong)可(ke)以調(diao)理氣(qi)(qi)(qi)(qi)機(ji),消除脹滿(man);白(bai)芍斂陰(yin)柔肝(gan),甘(gan)草(cao)緩肝(gan)之急(ji),“肝(gan)苦急(ji),急(ji)食(shi)甘(gan)以緩之”;芍藥與甘(gan)草(cao)合用(yong),可(ke)以調(diao)理肝(gan)脾,肝(gan)脾得和(he),氣(qi)(qi)(qi)(qi)機(ji)流暢,攣(luan)急(ji)可(ke)緩。

凡疏肝(gan)(gan)(gan)之(zhi)藥(yao)(yao),不(bu)宜溫燥,因肝(gan)(gan)(gan)為(wei)剛(gang)臟(zang)(zang),即(ji)內經所謂(wei)“將(jiang)軍之(zhi)官”。怒(nu)傷肝(gan)(gan)(gan),肝(gan)(gan)(gan)郁化火,陽易亢,陰(yin)(yin)易虧。但滋(zi)陰(yin)(yin)藥(yao)(yao)又礙脾,脾為(wei)土臟(zang)(zang)喜燥惡濕,肝(gan)(gan)(gan)氣犯胃(wei),每多化熱傷陰(yin)(yin),故用芍(shao)藥(yao)(yao)以(yi)柔肝(gan)(gan)(gan)抑(yi)肝(gan)(gan)(gan),平肝(gan)(gan)(gan)之(zhi)陽以(yi)益肝(gan)(gan)(gan)之(zhi)陰(yin)(yin)。肝(gan)(gan)(gan)氣平則脾不(bu)受侮,《本草綱(gang)目》所謂(wei)“土中瀉木(mu)”即(ji)指(zhi)此而言(yan)。

川楝(lian)子又名金(jin)鈴子,本草載(zai)入肝胃(wei)小腸(chang)經,性味苦寒。“泄(xie)肝止痛,治熱厥心(xin)痛”。《中藥學》謂:“川楝(lian)子有毒,成(cheng)人一(yi)次服6~8個即可(ke)引起頭暈、嘔吐、腹瀉、呼吸(xi)困難(nan)、心(xin)跳、震顫、痙攣等癥(zheng)。”而張老在臨證中以川楝(lian)子為主配合治療肝氣犯胃(wei)作痛,常(chang)重(zhong)用至30g(砸碎煎)。不僅未見有中毒反應,而且每獲卓(zhuo)效。香附、陳皮疏肝和胃(wei);白術健(jian)脾胃(wei),合之為治療肝胃(wei)不和之有效方劑。
