惟(wei)賢人上配(pei)天(tian)(tian)以養頭,下(xia)象地(di)以養足(zu),中傍人事以養五藏。天(tian)(tian)氣(qi)通(tong)(tong)於(wu)(wu)肺,地(di)氣(qi)通(tong)(tong)於(wu)(wu)嗌,風(feng)氣(qi)通(tong)(tong)於(wu)(wu)肝,雷(lei)(lei)氣(qi)通(tong)(tong)於(wu)(wu)心,谷氣(qi)通(tong)(tong)於(wu)(wu)脾,雨(yu)(yu)氣(qi)通(tong)(tong)於(wu)(wu)腎(新(xin)校(xiao)正(zheng)云:按千金(jin)方云,風(feng)氣(qi)應於(wu)(wu)肝,雷(lei)(lei)氣(qi)動(dong)於(wu)(wu)心,谷氣(qi)感於(wu)(wu)脾,雨(yu)(yu)氣(qi)潤於(wu)(wu)腎)。六經爲川(chuan),腸胃爲海(hai),九竅爲水注之(zhi)(zhi)(zhi)氣(qi)。以天(tian)(tian)地(di)爲之(zhi)(zhi)(zhi)陰陽,陽之(zhi)(zhi)(zhi)汗,以天(tian)(tian)地(di)之(zhi)(zhi)(zhi)雨(yu)(yu)名之(zhi)(zhi)(zhi)。陽之(zhi)(zhi)(zhi)氣(qi),以天(tian)(tian)地(di)之(zhi)(zhi)(zhi)疾風(feng)名之(zhi)(zhi)(zhi)。暴氣(qi)象雷(lei)(lei),逆氣(qi)象陽。
倪(ni)海(hai)廈釋(shi)義:是(shi)故唯有賢能之(zhi)(zhi)(zhi)人(ren)(ren)(ren),能知天(tian)(tian)道(dao)之(zhi)(zhi)(zhi)運行一(yi)如(ru)(ru)(ru)人(ren)(ren)(ren)之(zhi)(zhi)(zhi)頭部(bu),養頭而順天(tian)(tian)道(dao);下(xia)足如(ru)(ru)(ru)地(di)(di),以養地(di)(di)之(zhi)(zhi)(zhi)道(dao)來(lai)養足;中則(ze)從(cong)人(ren)(ren)(ren)事間知所進退來(lai)養五臓。天(tian)(tian)之(zhi)(zhi)(zhi)陽氣(qi)(qi)(qi)入通肺,地(di)(di)之(zhi)(zhi)(zhi)陰氣(qi)(qi)(qi)入通於(wu)(wu)咽,風氣(qi)(qi)(qi)通肝臓,雷(lei)電通於(wu)(wu)心,山(shan)谷(gu)之(zhi)(zhi)(zhi)氣(qi)(qi)(qi)入通脾臟(zang),雨氣(qi)(qi)(qi)通於(wu)(wu)腎臓。體(ti)內(nei)三陰三陽經絡為(wei)自(zi)然界之(zhi)(zhi)(zhi)河川(chuan),腸胃(wei)如(ru)(ru)(ru)大海(hai)之(zhi)(zhi)(zhi)能受納(na),人(ren)(ren)(ren)之(zhi)(zhi)(zhi)九(jiu)竅乃水溢出之(zhi)(zhi)(zhi)地(di)(di)。如(ru)(ru)(ru)用(yong)天(tian)(tian)地(di)(di)來(lai)比喻人(ren)(ren)(ren)體(ti)內(nei)陰陽之(zhi)(zhi)(zhi)作(zuo)用(yong),則(ze)可說雨就(jiu)如(ru)(ru)(ru)同人(ren)(ren)(ren)身上的汗屬陽,陽的氣(qi)(qi)(qi)化就(jiu)如(ru)(ru)(ru)同天(tian)(tian)地(di)(di)間之(zhi)(zhi)(zhi)疾風一(yi)様(yang),人(ren)(ren)(ren)之(zhi)(zhi)(zhi)暴(bao)怒(nu)如(ru)(ru)(ru)天(tian)(tian)地(di)(di)間之(zhi)(zhi)(zhi)雷(lei),故氣(qi)(qi)(qi)向上升(sheng)如(ru)(ru)(ru)同陽一(yi)樣。
故(gu)(gu)治(zhi)不(bu)法天(tian)之(zhi)(zhi)(zhi)(zhi)紀(ji),不(bu)用(yong)地之(zhi)(zhi)(zhi)(zhi)理,則(ze)災害至矣(yi)。故(gu)(gu)邪(xie)風之(zhi)(zhi)(zhi)(zhi)至,疾如風雨(yu)。故(gu)(gu)善(shan)(shan)大治(zhi)者(zhe)(zhe),治(zhi)皮(pi)毛,其(qi)(qi)(qi)(qi)次(ci)治(zhi)肌膚,其(qi)(qi)(qi)(qi)次(ci)治(zhi)筋脈,其(qi)(qi)(qi)(qi)次(ci)治(zhi)六府(fu),其(qi)(qi)(qi)(qi)次(ci)治(zhi)五藏。治(zhi)五藏者(zhe)(zhe),半死半生也。故(gu)(gu)天(tian)之(zhi)(zhi)(zhi)(zhi)邪(xie)氣(qi),感(gan)則(ze)害人(ren)五藏。水穀之(zhi)(zhi)(zhi)(zhi)寒熱,感(gan)則(ze)害於六府(fu)。地之(zhi)(zhi)(zhi)(zhi)濕氣(qi),感(gan)則(ze)害皮(pi)肉筋脈。故(gu)(gu)善(shan)(shan)用(yong)鍼(zhen)者(zhe)(zhe),從(cong)陰(yin)(yin)引(yin)陽(yang)(yang),從(cong)陽(yang)(yang)引(yin)陰(yin)(yin)。以(yi)(yi)(yi)右治(zhi)左,以(yi)(yi)(yi)左治(zhi)右。以(yi)(yi)(yi)我知(zhi)(zhi)(zhi)彼,以(yi)(yi)(yi)表知(zhi)(zhi)(zhi)裡。以(yi)(yi)(yi)觀過(guo)(guo)與不(bu)及(ji)之(zhi)(zhi)(zhi)(zhi)理,見微得過(guo)(guo)用(yong)之(zhi)(zhi)(zhi)(zhi)不(bu)殆。善(shan)(shan)診者(zhe)(zhe),察色(se)按(an)脈,先別陰(yin)(yin)陽(yang)(yang)。審(shen)清濁而(er)知(zhi)(zhi)(zhi)部分。視喘息,聽音聲,而(er)知(zhi)(zhi)(zhi)所苦(ku)。觀權衡規(gui)矩(ju),而(er)知(zhi)(zhi)(zhi)病(bing)所主。按(an)尺寸、觀浮、沈、滑(hua)、濇,而(er)知(zhi)(zhi)(zhi)病(bing)所生以(yi)(yi)(yi)治(zhi)(新校(xiao)正云:按(an)甲乙經作知(zhi)(zhi)(zhi)病(bing)所在以(yi)(yi)(yi)治(zhi)則(ze)無過(guo)(guo),下(xia)無過(guo)(guo)二(er)字續(xu)此爲句)。無過(guo)(guo)以(yi)(yi)(yi)診則(ze)不(bu)失矣(yi)。故(gu)(gu)曰,病(bing)之(zhi)(zhi)(zhi)(zhi)始起也,可刺而(er)已。其(qi)(qi)(qi)(qi)盛,可待衰而(er)已。故(gu)(gu)因(yin)其(qi)(qi)(qi)(qi)輕而(er)揚之(zhi)(zhi)(zhi)(zhi),因(yin)其(qi)(qi)(qi)(qi)重而(er)減之(zhi)(zhi)(zhi)(zhi)。因(yin)其(qi)(qi)(qi)(qi)衰而(er)彰之(zhi)(zhi)(zhi)(zhi)。形(xing)不(bu)足(zu)者(zhe)(zhe),溫之(zhi)(zhi)(zhi)(zhi)以(yi)(yi)(yi)氣(qi)。精(jing)不(bu)足(zu)者(zhe)(zhe),補之(zhi)(zhi)(zhi)(zhi)以(yi)(yi)(yi)味。其(qi)(qi)(qi)(qi)高者(zhe)(zhe),因(yin)而(er)越之(zhi)(zhi)(zhi)(zhi)。其(qi)(qi)(qi)(qi)下(xia)者(zhe)(zhe),引(yin)而(er)竭之(zhi)(zhi)(zhi)(zhi)。中滿者(zhe)(zhe),寫(xie)之(zhi)(zhi)(zhi)(zhi)於內。其(qi)(qi)(qi)(qi)有邪(xie)者(zhe)(zhe),漬形(xing)以(yi)(yi)(yi)爲汗。其(qi)(qi)(qi)(qi)在皮(pi)者(zhe)(zhe),汗而(er)發之(zhi)(zhi)(zhi)(zhi)。其(qi)(qi)(qi)(qi)慓(piao)悍(han)者(zhe)(zhe),按(an)而(er)收之(zhi)(zhi)(zhi)(zhi)。其(qi)(qi)(qi)(qi)實者(zhe)(zhe),散而(er)瀉之(zhi)(zhi)(zhi)(zhi)。審(shen)其(qi)(qi)(qi)(qi)陰(yin)(yin)陽(yang)(yang),以(yi)(yi)(yi)別柔剛。陽(yang)(yang)病(bing)治(zhi)陰(yin)(yin),陰(yin)(yin)病(bing)治(zhi)陽(yang)(yang)。定其(qi)(qi)(qi)(qi)血氣(qi),各(ge)守(shou)其(qi)(qi)(qi)(qi)鄉。血實宜決之(zhi)(zhi)(zhi)(zhi),氣(qi)虛宜脇引(yin)之(zhi)(zhi)(zhi)(zhi)。
倪(ni)海廈釋義:所(suo)以(yi)治(zhi)(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)法如(ru)不(bu)(bu)遵循天地(di)間之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)紀(ji)律,則(ze)(ze)(ze)災禍必(bi)(bi)立至(zhi)。天地(di)間之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)邪(xie)(xie)風(feng),來(lai)時(shi)快如(ru)風(feng)雨,所(suo)以(yi)最好之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)醫(yi)(yi)師(shi),能(neng)(neng)治(zhi)(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)始(shi)(shi)在(zai)(zai)皮(pi)(pi)(pi)毛(mao),再(zai)(zai)次之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)醫(yi)(yi)師(shi)是(shi)待(dai)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)入(ru)(ru)肌肉方察(cha)(cha)覺而(er)(er)(er)施治(zhi)(zhi)(zhi),更次的(de)(de)(de)則(ze)(ze)(ze)俟邪(xie)(xie)入(ru)(ru)經脈(mo)(mo)方去治(zhi)(zhi)(zhi)療,再(zai)(zai)次的(de)(de)(de)俟邪(xie)(xie)入(ru)(ru)腑後方察(cha)(cha)覺而(er)(er)(er)去治(zhi)(zhi)(zhi)療,最差的(de)(de)(de)是(shi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)已入(ru)(ru)臟(zang)(zang)(zang)再(zai)(zai)去設法補救的(de)(de)(de)。人(ren)(ren)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)至(zhi)五臓(zang)始(shi)(shi)治(zhi)(zhi)(zhi),生存(cun)機率只有一(yi)半矣。是(shi)故天氣(qi)(qi)不(bu)(bu)正常必(bi)(bi)傷人(ren)(ren)五臟(zang)(zang)(zang),水穀寒熱不(bu)(bu)適(shi)則(ze)(ze)(ze)必(bi)(bi)害(hai)消化(hua)系(xi)統(tong)。地(di)中(zhong)濕(shi)氣(qi)(qi)過(guo)(guo)盛,人(ren)(ren)感受之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi),則(ze)(ze)(ze)必(bi)(bi)損(sun)傷皮(pi)(pi)(pi)肉經脈(mo)(mo)。所(suo)以(yi)善於用(yong)(yong)(yong)(yong)(yong)針者(zhe),能(neng)(neng)從(cong)(cong)陰(yin)中(zhong)引陽(yang)出(chu)(chu),能(neng)(neng)從(cong)(cong)陽(yang)中(zhong)引陰(yin)出(chu)(chu),利用(yong)(yong)(yong)(yong)(yong)健側(ce)治(zhi)(zhi)(zhi)患側(ce),以(yi)穴位的(de)(de)(de)偵測而(er)(er)(er)知(zhi)(zhi)其(qi)(qi)(qi)(qi)(qi)他(ta)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)變,從(cong)(cong)四肢上的(de)(de)(de)穴位而(er)(er)(er)能(neng)(neng)探(tan)知(zhi)(zhi)內部(bu)臓(zang)腑之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)變。從(cong)(cong)而(er)(er)(er)察(cha)(cha)知(zhi)(zhi)其(qi)(qi)(qi)(qi)(qi)間過(guo)(guo)與不(bu)(bu)及之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)理,知(zhi)(zhi)進退(tui)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)機,始(shi)(shi)終(zhong)如(ru)一(yi)的(de)(de)(de)不(bu)(bu)怠(dai)慢。善於診斷(duan)的(de)(de)(de)醫(yi)(yi)師(shi),能(neng)(neng)察(cha)(cha)氣(qi)(qi)色按脈(mo)(mo),區別陰(yin)陽(yang)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)消長(chang),細(xi)審清濁之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)走向正常否,而(er)(er)(er)知(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)於何(he)部(bu)。看病(bing)(bing)(bing)(bing)(bing)(bing)(bing)人(ren)(ren)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)喘息,聽(ting)其(qi)(qi)(qi)(qi)(qi)發出(chu)(chu)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)聲(sheng)音如(ru)何(he),可知(zhi)(zhi)其(qi)(qi)(qi)(qi)(qi)為何(he)所(suo)苦。用(yong)(yong)(yong)(yong)(yong)陰(yin)陽(yang)五行之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)大(da)法規矩(ju)來(lai)判(pan)斷(duan)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)在(zai)(zai)何(he)處。手按寸、關、尺之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)脈(mo)(mo),察(cha)(cha)其(qi)(qi)(qi)(qi)(qi)為浮、沉(chen)、滑、濇等脈(mo)(mo)象之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)變化(hua),而(er)(er)(er)得知(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)所(suo)生,從(cong)(cong)而(er)(er)(er)治(zhi)(zhi)(zhi)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)。診斷(duan)不(bu)(bu)失(shi)(shi)則(ze)(ze)(ze)必(bi)(bi)無過(guo)(guo)矣。故可説,病(bing)(bing)(bing)(bing)(bing)(bing)(bing)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)始(shi)(shi)生,刺之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)皮(pi)(pi)(pi)毛(mao)立已,待(dai)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)勢盛時(shi),唯(wei)可待(dai)其(qi)(qi)(qi)(qi)(qi)衰(shuai)時(shi)方施治(zhi)(zhi)(zhi),所(suo)以(yi)若病(bing)(bing)(bing)(bing)(bing)(bing)(bing)輕則(ze)(ze)(ze)用(yong)(yong)(yong)(yong)(yong)發散之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)法,病(bing)(bing)(bing)(bing)(bing)(bing)(bing)重時(shi)減少用(yong)(yong)(yong)(yong)(yong)發散之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)法,以(yi)免(mian)陽(yang)過(guo)(guo)度流(liu)失(shi)(shi),病(bing)(bing)(bing)(bing)(bing)(bing)(bing)勢緩(huan)時(shi)則(ze)(ze)(ze)乘勢而(er)(er)(er)進。形衰(shuai)弱者(zhe),用(yong)(yong)(yong)(yong)(yong)溫(wen)藥補其(qi)(qi)(qi)(qi)(qi)氣(qi)(qi),精神不(bu)(bu)足者(zhe),必(bi)(bi)以(yi)厚味(wei)來(lai)補其(qi)(qi)(qi)(qi)(qi)五臟(zang)(zang)(zang),邪(xie)(xie)氣(qi)(qi)在(zai)(zai)上者(zhe),用(yong)(yong)(yong)(yong)(yong)汗(han)法發汗(han);邪(xie)(xie)居下者(zhe),則(ze)(ze)(ze)用(yong)(yong)(yong)(yong)(yong)攻下的(de)(de)(de)利尿(niao)劑;邪(xie)(xie)居中(zhong)部(bu)的(de)(de)(de),可用(yong)(yong)(yong)(yong)(yong)瀉(xie)藥攻內,使之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)盡(jin)(jin)出(chu)(chu)。始(shi)(shi)生之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)邪(xie)(xie),則(ze)(ze)(ze)用(yong)(yong)(yong)(yong)(yong)汗(han)法,因在(zai)(zai)皮(pi)(pi)(pi)毛(mao),可一(yi)汗(han)而(er)(er)(er)盡(jin)(jin)出(chu)(chu)其(qi)(qi)(qi)(qi)(qi)邪(xie)(xie)。若病(bing)(bing)(bing)(bing)(bing)(bing)(bing)邪(xie)(xie)已盛達四肢,則(ze)(ze)(ze)可用(yong)(yong)(yong)(yong)(yong)按摩方式(shi)來(lai)緩(huan)和。遇邪(xie)(xie)堅實者(zhe),先擊散再(zai)(zai)瀉(xie)出(chu)(chu)體外。治(zhi)(zhi)(zhi)病(bing)(bing)(bing)(bing)(bing)(bing)(bing)必(bi)(bi)細(xi)審陰(yin)陽(yang)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)消長(chang),分別其(qi)(qi)(qi)(qi)(qi)剛柔之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)性,務使邪(xie)(xie)盡(jin)(jin)而(er)(er)(er)不(bu)(bu)傷正,陽(yang)有病(bing)(bing)(bing)(bing)(bing)(bing)(bing)而(er)(er)(er)先固其(qi)(qi)(qi)(qi)(qi)陰(yin),陰(yin)有病(bing)(bing)(bing)(bing)(bing)(bing)(bing)則(ze)(ze)(ze)先固其(qi)(qi)(qi)(qi)(qi)陽(yang),不(bu)(bu)使病(bing)(bing)(bing)(bing)(bing)(bing)(bing)進,守(shou)定其(qi)(qi)(qi)(qi)(qi)氣(qi)(qi)血(xue)之(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)(zhi)流(liu)暢,不(bu)(bu)令(ling)其(qi)(qi)(qi)(qi)(qi)離經叛道。血(xue)液(ye)阻(zu)塞成實體,則(ze)(ze)(ze)必(bi)(bi)放其(qi)(qi)(qi)(qi)(qi)瘀血(xue),使其(qi)(qi)(qi)(qi)(qi)盡(jin)(jin)出(chu)(chu),不(bu)(bu)生阻(zu)礙。氣(qi)(qi)不(bu)(bu)足而(er)(er)(er)產生循環障礙,則(ze)(ze)(ze)用(yong)(yong)(yong)(yong)(yong)針來(lai)作導引,使氣(qi)(qi)足而(er)(er)(er)恢(hui)復正常循環。
